Health Savings Account (HSA)
An HSA is a tax advantage account that can help pay for qualified medical expenses. Acupuncture is an eligible medical expense. Therefore, Lambert Wellness does accept HSA payments.
Payment in full is due at the time of your service. We accept cash, check, and credit cards.
Privacy
All treatments, information and records will be kept confidential and securely stored for use by Lambert Wellness. Written consent must be given by me prior to any disclosure or sharing of my personal and clinical information with any third party.
Cancellations
Your appointment time has been set aside for you, and you may cancel or reschedule this appointment without penalty any time up to 24 hours before the start of your appointment. This allows other clients the chance to book in for your cancelled appointment time. If the appointment is canceled within 24 hours of that appointment time, we will charge a cancelation fee. If you are being charged a cancellation fee, you will be sent an invoice with payment instructions. No further appointments can be booked until the cancellation fee has been paid. If you have pre-paid for your cancelled appointment, including using a gift certificate as payment, your pre-payment will be forfeited.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your practitioner will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for payment of the full session that was booked.
NO-SHOWS
Failure to cancel your appointment or if you are more than 20 minutes late with no notice, it will be considered a no show. You will be charged for your full appointment and future service will be denied until payment is made. For prepaid appointments, no refund will be given.

COVID-19 POLICIES
Masks are required for both practitioners and clients throughout the visit.
By agreeing to your appointment time you are indicating that you have had no known contact with COVID-19 and are symptom free. Symptoms can develop quickly and we need to know if your appointment should be rescheduled. Please note, the cancellation fee may be waived should we need to reschedule your appointment if you are feeling unwell, so please let us know.
Consent to treat
By signing this policy I give the therapist consent to treat. Massage Therapy includes but is not limited to: the assessment and treatment of the soft tissues and joints of the body, using soft tissue manipulation, joint mobilization and hydrotherapy. Treatment plans will be discussed prior to the session start. I agree to let my therapist know if a technique is causing undue discomfort.
Skin care services will be agreed upon prior to treatment and may include but is not limited to: dermaplaning, ultrasound, steam, and exfoliation. I agree to inform my specialist if there are any changes to medication or skin conditions.
Sexual misconduct of any kind will not be tolerated and will result in the immediate termination of your service with no refund. Additionally you may be reported to the local authorities.
